It appears to be real. The bartender and the woman in the picture were commenters, and it looks pretty legit to me. I think it's great that coach is a man of the people, buying the whole bar drinks and taking photos with the crowd. Personally, my first thought is to be more critical of the woman in the picture. Who would drape themselves all over a Head Coach like that if they were nice enough to sit for a photo with them? She was definitely too cozy with him, and I can see the memes already.

I think it's okay to be a "man of the people" and pose for pictures, but this woman is sprawled out on the hood of a car really enjoying her "contact" with the coach of the Steelers. And Coach T doesn't seem to be minding it too bad either. If a picture of me popped up on some internet site with a tatoo'd woman (or any woman) draped all over me in short shorts on the hood of a car and I've got a look on my face like the one Tomlin has I would have some 'splainin' to do to the Mrs.
